Care Coordination For Medically Complex Babies
Little Heroes League helps babies born with multiple health challenges achieve their best life outcomes by providing care coordination that begins as early as birth in the Intensive Care Unit (ICU) and continues through hospital discharge to home. Together with the Ann and Robert H. Lurie Children's Hospital of Chicago, we are implementing a first-in-the-country program to provide care coordination for infants with medical complexity and their families to address this critical need in the hospital, at home, and in the community. An affiliated organization of Lurie Children’s, Little Heroes League was founded by the parents and grandparents of Livi, who spent over 150 days in the ICU in her first year of life spread over four visits that required three heart surgeries (two open heart), one intestine surgery, intensive vision and hearing tests, numerous sedations, and countless therapies.
